Children with VWD may also experience emotional and behavioural challenges in school and in their family life due to the fear of bleeding, and frequent bleeding may lead to absence from school.4 <\/sup>Moreover, the fear of bleeding may also present a substantial psychological burden<\/a> for the child\u2019s caregivers and prompt them to limit the physical and social activities that are important for the normal development of their child.5<\/sup><\/h5><\/p>\n<\/div>\n\n

Vloga dru\u017einskih zdravnikov in pediatrov<\/h3>\n<\/div>\n\n

VWD je najpogostej\u0161a motnja strjevanja krvi pri otrocih.6 <\/sup>Pogosti simptomi krvavitev pri otrocih z VWD vklju\u010dujejo hiter nastanek modric<\/a>, krvavitve v ustih in nosu<\/a> in podalj\u0161ane krvavitve iz manj\u0161ih ran in po kirur\u0161kem posegu.2 <\/sup>Kot dru\u017einski zdravnik ali pediater boste pogosto prvi zdravstveni delavec, ki bo videl otroka s takimi simptomi in va\u0161a naloga bo dolo\u010diti, ali ima otrok nenormalne vzorce krvavitev, ki zahtevajo nadaljnje preiskave pri specialistu.7 <\/sup>Tako za odraslega kot za otroka z mo\u017eno motnjo strjevanja krvi je jemanje anamneze krvavitev prvi korak pri diagnozi. Pri ocenjevanju anamneze krvavitev otroka je klju\u010dno, da se upo\u0161teva starost in razvojne sposobnosti bolnika. Dojen\u010dki, ki \u0161e niso mobilni, redko ka\u017eejo znake modric, lahko pa se jim pojavijo nenavadne krvavitve po obrezovanju penisa ali jemanju krvi.2,7 <\/sup>Ko otrok postane bolj mobilen, postanejo manj\u0161e praske in modrice zaradi udarcev ali padcev pogostej\u0161e. Vendar pa, \u010de so modrice ve\u010dje ali se pojavljajo pogosteje, kot je pri\u010dakovano, se lahko pojavi sum na motnjo strjevanja krvi. Pri otrocih z VWD se lahko pojavijo tudi modrice na nenavadnih mestih, kot sta hrbet ali trebuh. Take modrice se lahko zamenja za znak zlorabe in star\u0161e lahko skrbi zaradi te mo\u017enosti.8 <\/sup>In kon\u010dno, ko dekleta vstopijo v puberteto, postanejo mo\u010dne menstrualne krvavitve<\/a> najpogostej\u0161i simptom krvavitve VWD. Odlo\u010ditev o tem, ali so simptomi krvavitev nenavadni, je lahko izziv.7 <\/sup>Za prvo oceno simptomov krvavitev, \u0161e zlasti pri bolnikih, ki imajo na\u010drtovan kirur\u0161ki poseg, priporo\u010damo, da skupaj z bolnikom re\u0161ite na\u0161 test o krvavitvah<\/a>. Test bo pokazal, ali so indicirane nadaljnje preiskave – in lahko spremeni \u017eivljenje va\u0161ega bolnika!<\/p>\n<\/div>\n\n

Dokon\u010dna diagnoza VWD je rezultat izdelanega postopka.2<\/sup>Zato priporo\u010damo, da obi\u0161\u010dete najbli\u017eji zdravstveni center<\/a> za zdravljenje hemofilije, da izveste, ali so indicirani nadaljnji diagnosti\u010dni pregledi. Multidisciplinarne ekipe v zdravstvenem centru bodo tudi najbolje poskrbele za bolnike z motnjami strjevanja krvi.
